Service Coordination Program Children – Under 18 Years of Age (Dufferin County)

Our Service Coordination Program for Children under 18 years of age in Dufferin County, is a comprehensive program dedicated to providing essential service coordination and support to families and individuals diagnosed with intellectual or developmental disabilities, including those with an IQ at or below 70 and individuals on the Autism Spectrum.

Our approach is fundamentally family-centered, ensuring that we plan and coordinate a range of community programs, resources, funding opportunities, and supportive services tailored to the unique needs of each individual. We believe that every child deserves the opportunity to thrive, and our program aims to empower families by actively involving them in the goal-setting process.

Regular reviews of these goals will ensure that we adapt and grow alongside the individual, building their capacity and fostering independence. Service Coordination Program for Adults 18 Years of Age and Over (Dufferin County)

Our Service Coordination Program is available for adults with a documented developmental or intellectual disability*, in Dufferin County, facilitated through referrals from Developmental Services Ontario (DSO).

Our dedicated Service Coordinators are here to assist individuals in planning for their futures, helping them navigate the complex adult service system with ease and confidence.

Active participation by the adults we serve is a cornerstone of our approach, providing essential linkages to various services but also offer a solution-focused framework that supports both the individual and their family.

It is important to note that to be eligible for Ministry-funded adult services, individuals must apply through Developmental Services Ontario (DSO). We encourage all eligible individuals and their families to take advantage of these valuable resources and support systems available in our DCAFS community.

*Specifically, a sub-average general intellectual functioning with an IQ at or below 70 (2nd percentile or lower) and/or Autism Spectrum Disorder.
