  • Service Coordination Program

    The Service Coordination Program provides service coordination.

    The program supports families and individuals of all ages with a diagnosis of developmental disability which means ” a condition of mental impairment present or occuring in a person’s formative years (onset before age 18).” More specifically an IQ at or below 70 (within  the lowest second percentile). Exceptions are individuals diagnosed within the Autism Spectrum  Disorder.

    • The staff assess, plan, coordinate and help identify the needs of families and individuals and the community programs and supports they may be eligible to access.
    • The staff assist the family to access the appropriate formal and informal services and supports.
    • Service is tailored to the family/individual circumstances.
    • Families and individuals will have a full understanding of the services and supports available in their community and will be able to access them on their own or with minimal assistance.
    • It is outcome based with clear goals and actions identified with each client and/or family.
